Hospice & Community Care is dedicated to providing care and comfort to help patients and families live better at end of life, and provides hospice and palliative care, supportive care, and bereavement support for anyone in the community. Hospice is a specialized kind of care for people who have weeks or months, rather than years, to live and who are coping with any serious, life-limiting illness. Hospice provides expert medical care, pain and symptom management, emotional and spiritual support expressly tailored to the patient's needs and wishes. In 2016, Hospice & Community Care provided hospice care for 3,125 hospice patients, 2,494 palliative care patients, 118 supportive care patients, and 8,840 adults, teens and children received bereavement support through the Pathways Center for Grief & Loss. Support includes individual and group counseling, loss-specific bereavement support groups, workshop opportunities and camp experiences for children.

provide support for a Veteran who is receiving care from Hospice & Community Care through the organization's We Honor Veterans specialized program for Veterans who have served our country.
povide two days of hospice inpatient care for someone who is coping with pain and other difficult symptoms from serious illness, and needs the specialized medical and nursing support and care 24/7 at Hospice & Community Care's Inpatient Center.
provide five days of hospice care for an adult or child who is receiving care at home including medical care, help with managing pain and other challenging symptoms, emotional and spiritual support, and help from Hospice volunteers, entirely based on the patient and family's needs and wishes.
